1. You will need an email address.
2. Under Phase 1 b
3. "Click" on "individuals 75 and older" (highlighted in blue).

Phase I b Get the Details
• Individuals 75 and older
• Front line essential workers
• Individuals and staff in congregate settings
• Individuals between the ages of 65 and 74
• Individuals between the ages of 16 and 64 with comorbidities
4. Scroll down to the "Calendar" icon (highlighted in blue).
5. "Click" on Vaccine Administration Management Systems (VAMS)

6. Form Instructions
7. Fill out this simple form to register with the Vaccine Administration Management System (V AMS). You will need to enter your full name, date of birth, zip code, occupation (including "retired"), and email address. The information you enter will only be used for purposes of scheduling your vaccination, and will be kept private.
8. Once registered, VAMS will email you to guide you through the appointment process for receiving the COVID-19 vaccine.
9. At this time, ONLY people eligible under Phase la or who are age 75 and over may register here. Please only fill out this form if you are eligible to receive the vaccine.
10.Complete form and Click "SUBMIT"
